Resident Dr. Eisi Mollanji (“Mollanji”) committed the first set of HSC Psychealth crimes when he:

• chose to reverse my voluntary status by illegally signing a Form 5 on 20240916 and
• created a forged document by signing a pre-signed “Notification of Status”.

A Form 5 “Change of Status” must be signed by the attending physician pursuant to Manitoba’s Mental Health Act (“MHA“) subsection 6(1)  ; not merely a resident physician.

I suspect this crime was committed so Enns could avoid violating the MHA‘s s. 16(2) by signing both the Form 5 and the Form 6 (which solidified my illegally-obtained, involuntary detention) herself. Then, after I informed Mollanji that my detention was illegal, he attended multiple interviews held by Dr. Jessica Enns  (“Enns”) and Dr. Matthew Buchok (“Buchok”); and, despite hearing my testimony and having access to all of my evidence labeled s. 1 to s. 12, he did nothing to stop Enns’ or Buchok’s mental torture, continuous threats or illegal detention of me (establishing liability pursuant to s. 21(1) Party to Offence of the Criminal Code of Canada (“CCC“).
